A school's incident reporting system keeps track of all incidents that occur to pupils during the course of the day, no matter how minor they might be. The parent receives a copy of the incident report, which includes the location, witness information, treatment, and other details.

In what format should a school incident report be written?

Find out what the facts are:

Identifying information about the child.
When and where the incident occurred.
Describe the type of injury.
If therapy or first aid were administered, please provide details.
Identify any witnesses.
Identify the person who treated the injury by their name and signature.
A description of the accident and the signature of the person who reported it.
A signature from the parent is required.

In what ways do school incidents occur?
